Community Nurse (RPN)
Full-Time (1 Year Contract)

(Monday – Friday)

The Community Nurse, (Clinical Nurse Specialist) will work collaboratively across Durham Region supporting clients of the Adult Day Programs (ADP), Supportive Housing (SH), and Assisted Living Services (ALS) and other Community Support Services (CSS) to provide rapid front-line nursing response to older adults living in the community who are at risk, or who have experienced a gerontological event that may result in the loss of independence, Emergency Department visit, hospitalization or a move to a long-term care setting without timely intervention.

Responsibilities include but are not limited to:

  • Receiving requests and attend to needs of adults living in the Ajax/Pickering service areas, and provide interventions as appropriate
  • Providing health care navigation assistance by collaborating and coordinating care with Most Responsible Practitioner (MRP) and other health care professionals
  • Performing assessment activities, including conducting client interviews, comprehensive physical examinations and assessment of psychosocial, cultural, and ethnic factors affecting client needs
  • Providing health teaching and education to clients
  • Collaborating with CCD staff (service coordinators, supervisors/managers, etc.,) to evaluate and adjust care plans to reflect clients’ care needs
  • Communicating plan of care to the client/family and members of the healthcare team
  • Documenting clinical findings
  • Participating in regular data collection of the nursing services being provided to CCD clients
  • Practicing within the guidelines stipulated in the infection control policies

Qualifications required:

  • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
  • Membership with the RPNAO preferred
  • Minimum of 2-3 years’ recent experience in adult nursing – assessing, treating, and developing care plans for older adults
  • Recent community and/or primary care experience with adults preferred
  • Extensive clinical knowledge and skill as they apply to the adults
  • Experience with geriatric mental health, cognitive impairment, and dementia
  • Proficient in Microsoft Office and ability to learn host agency AlayaCare
  • First Aid Certification with HCP Level CPR training
  • Satisfactory Police Vulnerable Sector Check
  • Sensitivity to adults with needs related to aging, physical and/or mental health
  • Adherence to COVID-19 safety practices including two doses of COVID-19 Vaccine (where medically able in accordance with our Immunization Policy)
  • Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation: Starting at $27 per hour; 6% vacation pay

* Pay will be commensurate based on education and experience
