Registered Practical Nurse / Client Coordinator

Quest Community Health Centre is a not-for-profit, interdisciplinary, health care organization.   Our holistic model of care focuses on primary care, illness prevention, health promotion, community capacity building.

With your skills, positive outlook and clinical expertise, you are a catalyst who will provide person-centered care services for various priority populations including Seasonal Agricultural workers who commonly experience barriers to accessing healthcare. The RPN/CC works as part of the health care team  focused on  health promotion, disease prevention, and community capacity building and works to the full scope of the RPN professional standards within a harm reduction framework.

This position plays a vital role in ensuring effective coordination of client care, including linking clients to resources. It also provides administrative support in taking clients, schedules appointments/tests, drafts correspondence/referrals and maintains electronic charts and files.

The RPN/CC works with a high performing interdisciplinary team of professionals whose objectives are to provide health care and follow-up during clinics and through outreach to farms. Hours of work are from 11:30am -7:30pm and include Sundays during the growing season.

Your qualifications include Current and valid registration with College of Nurses of Ontario as an RPN. A valid Ontario driver’s license, access to reliable transportation and a Police Reference check are required and NVCI or willingness to obtain certification.  Fluency in Spanish is an asset.
