Job Description: Dorval Medical Family Health Team

Registered Practical Nurse – Permanent Part-time 0.6 FTE

 

Main Duties and Responsibilities

Complete individual patient assessment and goal directed nursing care within a primary care setting, including – health assessment, immunizations, ordering laboratory-screening tests, diagnosis, treating illnesses and follow-up, health promotion and disease prevention counseling, support for chronic disease management and patient navigation.

Book specialist referrals and appointments in consultation with physician.

Perform telephone triage and assessment of patients  

Respond to clinical/administrative tasks that are ordered by Physician/NP

Provide telephone advice/results/ instructions/appointment information to patients

Manage home care referrals and book diagnostic appointments as ordered

Perform child/adult immunizations and other medically authorized injections

Participate in flu shot and Public Health directed vaccine programs

Assist in well-baby visits

Provide wound care and suture removal

Assist in performing medical/surgical procedures

Support/assist lab technician with venipuncture and ECG

Manage specimens, sterilize equipment and re-stock medical supplies as required

Support other clinical activities within the primary care office setting, including wellness care, health promotion, and prevention activities.

 

Skills & Knowledge Requirements

Minimum of 2 years clinical experience, preferably in a primary care setting

Excellent oral and written communication skills

Flexible, self-directed, and compassionate

Ability to work co-operatively with all members of the health care team and function effectively within an inter-professional environment

Participate in problem solving and share ideas and concerns with the health care team

Ability to prioritize tasks and manage time efficiently

Knowledge of and working experience with computer systems and applications

Required experience using an EMR, with expertise in Accuro an strong asset

 

Education and Specific Job Requirements

RPN license and certification with the College of Nurses in Ontario

Current membership in appropriate professional associations (RPNAO)

Current basic cardiac life support certification

Vaccines (COVID-19 and others) are a requirement of the job unless you have an exception based on medical or grounds pursuant to the Ontario Human Rights Code

Understanding and adherence to IPAC best practices (i.e. masking as required) and all Public Health mandated activities

 

 

Accountability

The RPN reports to and is responsible to the Executive Director. All clinical decision-making outside the scope of the RPN must be done in consultation with the NP and/or affiliated physicians.   

 

Preferred Hours of Work

Wednesday 8:30-5:00 p.m.,

Thursday 11-7p.m. (evening shift)

Friday 8:30-5:00 p.m.

*Additional shifts available for vacation coverage on Mondays and Tuesdays

 

Compensation

 

$28.00 to $29.50 per hour based on experience, plus a generous vacation allowance
