Client Care Coordinator – Day Hospice Program

Permanent Part-time (10 hours/week with potential to increase)

 

Are you experienced in hospice palliative care and looking for a different kind of practice environment? Our ideal candidate is an RN or RPN who has an understanding and desire to create an environment rich in sharing and support for the Day Hospice Program (DHP) clients. We are seeking someone who possesses a strong skill-set in the areas of communication, group facilitation/moderation, and programming. The candidate will work closely with staff and volunteers to create an environment that fosters genuine compassionate connection.

 

RESPONSIBILITIES:

Overall responsibility for clients attending the DHP including assessment, care plan development, client orientation and documentation, connecting with clients outside of program time, support to caregivers and program discharge. Collaborate with Program Coordinator for program planning and take the lead on program facilitation/implementation and evaluation. Supervise and support volunteers and/or students.

 

QUALIFICATIONS:

  • Post-secondary degree/diploma in nursing. Current registration in good standing with Ontario College of Nurses
  • CPR & First Aid Certification
  • Experience working in hospice palliative care &/or community environment is an asset
  • Exceptional communication and interpersonal skills
  • Ability to work unsupervised exercising a high degree of discretion and independent judgement
  • Outstanding organizational, time management, and problem solving skills

ABOUT THE DAY HOSPICE PROGRAM:

The Day Hospice Program is designed for those diagnosed with a progressive life-threatening illness. The Client Care Coordinator and trained volunteers provide care and support to clients, in a home-like setting. Programming includes wellness opportunities, art/creativity, guest speakers, complementary therapies and more. Refreshments and lunch are provided. Benefits include connection, emotional support, education & caregiver respite. The program runs on Thursdays at our Richmond Hill location.

Our professional staff and network of dedicated volunteers provide our community with innovative programs and access to information. We are not a residential hospice but instead provide support to people in their homes, in the community and at our two locations.
