<p>Scarborough Centre for Healthy Communities (SCHC) is dedicated to meeting the diverse, holistic health needs of the people of Scarborough by addressing the physical, mental, social, financial and environmental aspects of their health.&nbsp; Through the promotion of healthy lifestyles and delivery of a comprehensive range of culturally competent health and social services, we cultivate vital and connected communities.</p> <p>&nbsp;</p> <p>We are currently looking to hire a:</p> <p>&nbsp;</p> <p><strong>Community Health Registered Nurse </strong></p> <p><strong>&nbsp;Community Setting (Toronto Community Housing) &nbsp;</strong></p> <p>1 Full time, permanent position&nbsp;</p> <p>&nbsp;</p> <p><strong>Reporting To: &nbsp;</strong><em>Community Engagement Manager </em></p> <p>&nbsp;</p> <p><strong>Purpose:&nbsp; </strong>The Community Health Registered Nurse will provide primary health care services within an inter-professional model of care focused on the social determinants of health with an emphasis on prevention, health promotion and chronic disease management for vulnerable complex clients with a history of mental health and addictions living in Scarborough community.</p> <p>&nbsp;</p> <p>The Community Health Registered Nurse will support frontline programs in the community through nursing and health-related education. The RN will provide three days of support to the Lawrence East Partnership Program, a collaborative service for residents who are vulnerable and have complex needs living at two Toronto Community Housing buildings, 4175 and 4205 Lawrence Avenue East in Scarborough.&nbsp; Additionally, the RN will provide support at numerous community programs including but not limited to the Social Support programs, Assisted Living seniors program and Adult Day Centre.</p> <p>&nbsp;</p> <p><strong>Key areas of responsibility:</strong></p> <p>&nbsp;</p> <p><strong>Scope of Practice:</strong></p> <ul> <li>Knowledge of trends, issues related to the delivery of services to people living with mental health issues, addictions and chronic conditions.</li> <li>Excellent knowledge of community resources available to people living with mental health issues, addictions and chronic conditions.</li> <li>Work and communicate other primary care providers (physicians, nurse practitioners) to support individuals with ongoing care</li> <li>Physical assessment skills</li> <li>Perform clinical procedures and treats minor episodic illnesses within scope of practice</li> <li>Participates in chart audits, client case conferences and huddles</li> <li>Ensures medical equipment is operational and stocks are maintained</li> <li>Perform new client intake, client navigation and service coordination</li> <li>Perform triage and follow up care for pre-scheduled or walk in clients, conduct community and home visits</li> <li>Provide direct practical support, coaching and counsel to clients in their pursuit of identified goals and needs</li> <li>Organizes, implements and evaluates harm reduction (needle exchange, HIV point of care testing, safe sex, family planning and safe drug use counselling)</li> <li>Smoking cessation counselling</li> </ul> <p>&nbsp;</p> <p><strong>Health Promotion: </strong></p> <ul> <li>Identify needs of clients/community and assists in the development of programs and services as required</li> <li>Complete outreach and community work through program planning, development, implementation and evaluation</li> <li>Community development and partnership development as required</li> <li>Provide health education</li> </ul> <p>&nbsp;</p> <p><strong>Professional Practice: </strong></p> <ul> <li>Maintains accurate and complete electronic documentation</li> <li>Participates&nbsp; in inter-professional policy and procedure development</li> <li>Ongoing participation in education workshops to enhance skills and knowledge</li> <li>Participates in the accreditation process and the implementation of strategic plans</li> <li>Participate in data collection and Quality Improvement initiatives.</li> <li>Designing and delivering group programs as needed.</li> <li>Perform other duties as assigned that are reasonable within the scope of role</li> </ul> <p>&nbsp;</p> <p><strong>Educational and/or Professional Qualifications:</strong></p> <ul> <li>Undergraduate degree in nursing from an accredited university</li> <li>Registration and in good standing with the College of Nurses of Ontario</li> <li>Member of RPNAO, preferred</li> <li>Venipuncture certification (or willingness to complete)</li> <li>Certificate/Diploma in Community Health Nursing or Mental Health Nursing is considered an asset</li> <li>Current HCP CPR</li> </ul> <p>&nbsp;</p> <p><strong>Level of Experience:</strong></p> <ul> <li>Minimum of two (2) to five (5) years direct client care</li> <li>Community based mental health nursing is an asset</li> <li>Geriatric nursing is an asset</li> </ul> <p>&nbsp;</p> <p><strong>Skills and Attributes:</strong></p> <ul> <li>Demonstrated experience/confidence in working independently, with high degree of self-direction and strong organizational skills</li> <li>Excellent interpersonal, communication, decision making, problem solving, organizational, conflict management and time management skills</li> <li>Ability to prioritize needs and work efficiently</li> <li>Excellent diplomacy and conflict resolution skills</li> <li>Excellent computer skills (e.g., Word, Excel, Microsoft Outlook)</li> <li>Excellent oral and written skills</li> <li>Ability to manage adapt positively to an ever changing health care/work environment</li> <li>Familiarity or willingness to practice with electronic health record documentation environment</li> <li>Ability to work within&nbsp;
