Community Nursing Support - Congregate Settings RN/ RPN (3 positions)

The Dufferin Area Family Health Team has an immediate opening for 3 RNs/RPNs to support the local congregate settings in Dufferin County to meet patient care needs during COVID. Working alongside community partners and a multidisciplinary team, the RN/RPN will provide direct care to residents as needed. Supported by DAFHT affiliated family physicians locally, the Community Nurse will liaise with setting staff and provide direction to other care providers as appropriate.  The position(s) will support a variety of 12-hour shifts, including day, night and weekend coverage.

What you’ll do:

  • Assess & assign care according to residents’ needs, physician’s orders, nursing standards and available resources, and develop or update documentation, reports and other records
  • Provide & plan individualized care
  • Deliver medications/treatments as required
  • Ensure all MOHLTC requirements are met
  • Other duties as assigned.

Education/Experience Requirements:

  • All Registered Nurses and Registered Practical Nurses must have a current Certificate of Registration from the College of Nurses of Ontario and be a member in good standing
  • Bachelor of Science in Nursing preferred
  • Minimum 1 year experience in Long Term Care is preferred
  • Gerontology certificate or related course an asset
  • Maintains competence and knowledge relevant to current nursing practices
  • Current BCLS certification
  • COVID-19 IPAC fundamentals https://www.publichealthontario.ca/en/education-and-events/online-learning/ipac-fundamentals

Other Skills/Abilities:

  • Satisfactory criminal reference check, including Vulnerable Sector Screening
  • Strong communication and interpersonal skills
  • Ability to manage multiple tasks simultaneously
  • Organized and able to prioritize, manage time effectively and be flexible in a very active work environment
  • Team player with a willingness to assist others
  • Previous experience working in a congregate setting
  • Previous experience providing patient care during COVID
  • Professional and courteous manner
  • Proficient in the use of computers with experience in programs for Long Term Care Homes
